  • Only providing Pfizer bivalent additional doses to established HHCMG primary care patients (ages 12+).
  • Certain HHCMG pediatric offices will provide the Pfizer bivalent vaccine, if applicable, to children under 11 years old.

Schedule Your Vaccine

Everyone age 6 months and older is eligible for a COVID-19 vaccine:

  • Those under 18 need to be accompanied by a parent or guardian. If you do not have proxy access to your child(ren)’s MyChartPLUS account, the parent or legal guardian of the child being vaccinated will need to complete a proxy form at the clinic site. Forms will be available, but you can save time by downloading the proxy form and taking the completed form with you to the clinic. DO NOT SCHEDULE A PEDIATRIC VACCINE APPOINTMENT ON YOUR OWN MYCHARTPLUS ACCOUNT.

Primary Vaccination Series:

  • For children 6 months-4 years old a 3-dose bivalent series is recommended. Dose 1 and 2 given 3-8 weeks apart and Dose 3 given at least 8 weeks after second dose.
  • For children 5-11 years old a single bivalent pediatric dose is recommended
  • For adults 12+ years old a single bivalent adult dose is recommended
  • For immunocompromised individuals, a 3-dose primary series is recommended

Additional Doses:

  • For children 6 months-4 years old a single bivalent pediatric dose is recommended 2 months after completion of the primary series only if all 3 doses were monovalent doses (series completed before 12/9/22)
  • For children 5-11 years old a single bivalent pediatric dose is recommended 2 months after the last monovalent dose
  • For adults 12-64 years old a single bivalent adult dose is recommended 2 months after the last monovalent dose
  • For adults 65+ years old one additional bivalent adult dose may be given at least 4 months after the initial bivalent dose. Total of 2 bivalent doses allowed.
  • For immunocompromised individuals a single additional bivalent dose may be given at least 2 months after the initial bivalent dose. Additional doses may be administered at the discretion of a healthcare provider based on the individual's clinical circumstances.
