Schedule Your Vaccine
Everyone age 6 months and older is eligible for a COVID-19 vaccine:
- Those under 18 need to be accompanied by a parent or guardian. If you do not have proxy access to your child(ren)’s MyChartPLUS account, the parent or legal guardian of the child being vaccinated will need to complete a proxy form at the clinic site. Forms will be available, but you can save time by downloading the proxy form and taking the completed form with you to the clinic. DO NOT SCHEDULE A PEDIATRIC VACCINE APPOINTMENT ON YOUR OWN MYCHARTPLUS ACCOUNT.
- For more information regarding COVID-19 vaccines, visit the CDC's COVID-19 vaccine site.
