What happened:

Telemetry nurse Mary-Lee Clark was precepting a new nurse when she realized the policy for IV medication was incorrect on PolicyTech.

Clark went to PolicyTech to validate and verify the dose of an IV medication and found it lacked many medications that had been part of the previous version. She alerted the right people to have it corrected, not only modeling high-reliability behavior by checking PolicyTech but also using attention to detail and ARCC-ing the situation up so it could be remedied.

Clark demonstrated a questioning attitude and paid attention to detail. This led to using cross checks, validating and verifying to make sure the medication was correct.
