What happened:

Reviewing a discharge plan, Larry noticed there was some confusion in the order for a discharge medication. He called the provider to discuss and was able to provide an easy-to-use dose pack of the medication and clearer instructions for the patient. He greatly improved the likelihood the patient would be able to correctly take needed medication once out of the hospital.

Safety behaviors demonstrated:

Larry practiced a questioning attitude and used the HRO behavior of ARCC-ing up. He had concerns about the patient’s ability to understand the discharge medication instructions. He escalated his concerns up the appropriate chain of command and had a courageous conversation, leading to a better discharge for the patient.
