What happened:

A psychiatric patient was on five-minute checks, but when the staff completed the 8:10 p.m. checks, John followed his instincts and looked in on the individual two minutes later. The patient was found attempting self-strangulation using a hospital robe. Three staff members removed the robe and the patient was evaluated in the Emergency Department.

Safety behaviors demonstrated:

John’s attention to detail and sound instincts prevented potential harm to the patient.
