Vennada had a patient with a note stating the individual had a below-the-knee amputation. She quickly observed that both the patient’s legs were fully intact. She found additional paperwork that copied this incorrect note. The patient’s information was updated, ensuring there would be no further misunderstanding of the individual’s status.
