What happened:

While assisting in a percutaneous tracheostomy on a paralyzed and sedated patient, Bill immediately recognized that the newly placed trach was malfunctioning and moving out of the airway. He quickly pushed it in place and alerted the physician. The faulty trach was replaced without any complications. Bill’s fast response ensured that the patient experienced no loss of airway and led to a safe outcome.
