What happened:

Charlene, reviewing a recent discharge, noticed that several of her patient’s medications were missing from the active orders. She realized that these missed prescriptions may have been leading to some of the patient’s symptoms. She ARCC’d her concerns to Abby, who agreed with Charlene’s findings. The physician was notified, the drugs were reordered and the patient was supplied with all needed medications.

 

Safety behaviors demonstrated:

Charlene was attentive to detail and practiced STAR (Stop-Think-Act-Review) behaviors in a way that benefited the patient.
