What happened:
While performing a magnified visual inspection of a surgical instrument after it had been cleaned after use, Celeste Milne found that its tiny stainless steel tip was missing. She immediately alerted the SPS Site Manager, who confirmed the finding and followed up with Perioperative Clinical Leadership, which determined that there was no patient impact. The instrument was removed from service.

Celeste’s keen attention to detail (STAR), stopping the line, practicing 200 percent accountability and ARCCing her concern to leadership led to the removal of a defective piece of equipment.
