Good Catch Champion: Diane Bowler
July 07, 2022
Diane Bowler
CT Technologist
Charlotte Hungerford Hospital
What happened:
When reviewing an order for a CT scan with contrast, Diane noticed that all prior scans on the patient had been done without contrast. She contacted the ordering provider, who told her there was no problem with contrast. In addition, there were no allergies documented in EPIC. But in reviewing the final contrast questionnaire, Diane noted that the patient had written something illegible. She communicated with the patient in writing (the individual was hearing-impaired) and was able to confirm that the patient had an allergy to intravenous “cat scan dye.” She followed up with the provider and the test was cancelled. She also made sure the allergy was added to EPIC.
Safety behaviors demonstrated:
Diane’s questioning attitude, attention to detail and clear communication helped identify a serious patient allergy.
