What happened:

Caitlyn recognized that an order for tranexamic acid (TXA) – a drug used to control bleeding – was being directed to the wrong patient. She reviewed the patient’s notes and saw that the individual was stable on apixaban, a drug that helps prevent blood clots. Caitlyn checked with the team and found that TXA was meant for a trauma-unit patient instead. Her questioning attitude and thoroughness ensured that each patient got the correct medication.
