What happened:

The patient care team of Kristin, Jenny and Tara recognized difficulty while suctioning a patient’s tracheostomy. Kristin, Jenny and Tara quickly assessed the patient’s airway and immediately escalated their concern to Dr. Olufunmilayo Olubukola Falade, the intensivist. Dr. Falade identified the cause as an airway obstruction and replaced the tracheostomy tube. The patient was stabilized and remained on the unit.

 

 

Safety behaviors demonstrated:

The team practiced 200 percent accountability and a questioning attitude. By ARCC’ing up the concern they were able to get the appropriate expert to the bedside and resolve the issue safely.
