Many accolades from state hospital group

Members of the East Region Community Health Management Program were honored, shown here with East Region President Donna Handley, second from right and Jeff Flaks, HHC president and CEO, far right. 

Hartford HealthCare colleagues and teams earned a variety of awards at the recent Connecticut Hospital Association (CHA) annual meeting. In addition to recognizing Healthcare Heroes from each hospital, announcements included:

• Election of Jeff Flaks, Hartford HealthCare president and chief executive officer, as chairman of the CHA Board of Trustees.

• Community Service Awards for the Community Health Case Management Program at Backus and Windham hospitals, an award from the CHA and state Department of Public Health. The program, operated jointly by the East Region hospitals, provides an innovative, community-based care coordination model that reaches vulnerable populations in nonclinical settings and connects people to critical health and social services.
