Social Support

Providing multifaceted support for patients and families is part of what we do every day at the Chase Family Movement Disorders Center and social workers help make that happen.

As part of the patient-centered model, the Chase Family Movement Disorders Center offers onsite social work support and wellness services for patients and their caregivers. Our social workers are specialized and qualified to help you, your loved ones, and your care partners adapt to the emotional, physical, and practical concerns of living with Parkinson’s and other movement disorders. We provide comprehensive services such as:

  • Connecting patients with community resources and behavioral health providers.
  • Applying for state and federal assistance programs including Medicare and Medicaid.
  • Legal processes to establish durable power of attorney and/or conservatorship.
  • Helping families and patients to understand the various levels of care. Such as assisted living, memory care, skilled nursing facilities, and deciding which setting is best for them.
  • Individual, couples and group psychotherapy.

Parkinson’s Disease Education Classes

The Parkinson’s Roadmap for Education and Support Services™ support group program is an 8-week pilot program we offer several times a year for people who have been diagnosed with Parkinson’s disease within the last 5 years. Discussion topics include current research about Parkinson’s disease, falls prevention, coping skills, healthy sleep hygiene, maintaining relationships, and how physical, occupational, and speech therapy can help you.
