Survivorship Program

Our survivorship program helps patients transition from the end of active cancer care to optimal life-long wellness.

As part of a multidisciplinary, integrated team, the Survivorship Program provides comprehensive, coordinated after-care for cancer patients treated at Hartford Healthcare.

The goal of the Survivorship visit is to develop and deliver a comprehensive, effective and meaningful care plan to each survivor.

The services provided by the survivorship program include:

  • A personalized treatment summary and survivorship care plan that will be shared with all your medical providers
  • Follow-up surveillance recommendations based on your cancer diagnosis
  • Education on prevention, detection, and possible late term cancer treatment complications
  • A list of community and national resources that address survivorship issues
  • Information regarding post-cancer nutrition and exercise guidelines
  • Identification and recommendations for any physical and/or psychosocial effects of your treatment
  • Opportunity for referrals to other services which include: nutrition, genetic counseling, smoking cessation, psychological counseling, social services, physical therapy, and integrative therapies such as massage and reiki.

In addition, by encouraging active patient and caregiver participation in the plan of care, post-treatment anxiety may be reduced and quality of life enhanced for Hartford Healthcare's cancer survivors.
