Iora to deliver innovative primary care to Hartford HealthCare Employees and families
Hartford HealthCare and Iora Health today announced a partnership to bring Iora Health’s innovative primary care delivery system to Hartford HealthCare employees and their families.
As like-minded leaders in innovative health care, the two organizations are working together to continue to improve the way patients receive primary care. This new relationship includes the opening of a new primary care practice, built with Iora Health’s innovative care delivery system, for Hartford HealthCare employees and their families.
The practice, called the Hartford HealthCare Health Center, Primary Care by Iora Health, has been designed specifically for the Hartford HealthCare community. Iora Health’s primary care operating model includes several unique features: integrated primary care teams that include providers, health coaches and a behavioral health specialist; longer appointment times with the care team to better understand patient needs; a proprietary collaborative care platform that is built with the patient in mind; and exceptional service within — and beyond — the walls of the practice to help patients navigate the often complex health care system.
